Choose a principle of healthy ageing outlined in week 4's lecture. Choose a modifiable risk factor outlined in lecture 2. Your OLDER ADULT patient is currently admitted to an acute care facility, but being discharged later today. Propose a practical and feasible way to promote the principle of healthy ageing during everyday interactions with your fictional older adult patient. Include in your post: 1) A brief outline of your fictional patient: name, sex, age, their functional status, the reason for admission, the patients discharge location, and their modifiable risk factor (70 words max.) For example: In my care in Mrs Mary Hart, an independent 87 year old lady currently admitted to the ward with pneumonia. Mary is being discharged today, and during these discussions Mary mentions that she is going to miss being on the ward, because ‘it’s a bit lonely at my house, with no-one to talk to’ 2) An explanation of the effects of the modifiable risk factor, specifically for older adults and linked to your patients story. (100 words) 3) An explanation of the healthy ageing principle, including an outline of what how healthy ageing would be affected without it, with reference to your chosen modifiable risk factor. (230 words) HINT: Please keep in mind that a superficial “encouraging Mrs Mackay to eat well” or “explaining the benefits of eating well” would not be an appropriate promotion, nor is it the intended purpose of this assignment.
